Graceway Pharmaceuticals, LLC has become aware that, due to an administrative error, Maxair Autohaler (pirbuterol acetate inhalation aerosol) is not presently covered on a small number of Medicare Part D plans.

We are working closely with the Centers for Medicare and Medicaid Services (the government agency that manages Medicare), as well as insurance companies to resolve this problem.

To help you until the product is covered, we are providing FREE sample units of Maxair Autohaler at no cost to you.

To start the process of getting these free product samples, please call the Graceway Pharmaceuticals Customer Service Department at 1-800-328-0255 and select the number "9" after the voice prompt. Please note that you will be asked to provide the following information when you speak to the Customer Service Representative:

1. Your full name, street address, and telephone number.
2. Your health care prescriber's name, street address, and telephone number.

You will be given instructions by the Customer Service Representative on how and when to contact your prescriber to obtain your free Maxair Autohaler samples.

Indication:

Maxair^® Autohaler^® is indicated for the prevention and reversal of bronchospasm in patients 12 years of age and older with reversible bronchospasm including asthma. It may be used with or without concurrent theophylline and/or corticosteroid therapy.

Safety Information:

WARNINGS: The effects of Maxair Autohaler may last 5 hours or longer. Therefore, it should not be used more frequently than recommended. Do not increase the number or frequency of doses without speaking with the prescribing physician. If the recommended dosage does not provide relief of symptoms, or your symptoms get worse, speak with your physician. While taking Maxair Autohaler, other inhaled medicines should not be used unless prescribed.
